Lujsdal. An example of integrating Social and Health Care.


FOR MY ENGLISH READERS HERE IS  an entry in ENGLISH!
Ljusdal is a little and beautiful village located in Halsingland. The closest hospital is 60 km away.
In Ljusdal, in the SAME BUILDING, different Services are concentrated, one is HEALTH CARE oriented , financed by the County and the other for SOCIAL CARE, financed by the Municipality.
For HEALTH CARE SERVICES, there is a Health Center, an Emergency Unit 24h/365d, 10 hospitalization beds, some specialist consultations and a Rehabilitation Center.
For SOCIAL SERVICES, there is a Nursing Home with 24 rooms and 10 beds of Social Hospitalization. They are financed by the Municipality, the user only pays for food and the laundry.
The Nursing Home has INDIVIDUAL ROOMS with an ensuite BATH ROOM, and as stipulated by Law, they also have a small Kitchen. It is located on the ground floor with two common kitchens and a larger dining room with outside access to the gardens.

The Rehabilitation Center has 3 levels, the downstairs is for those who are most disabled with a pool, the second for occupational therapy and the third has a reception, a large gymnasium and individual physiotherapy treatment rooms.  They have a very welcoming room for the first visit decorated in warm, soft colors.

The combination of all of these resources in the same location, makes it very easy for the patient to receive a seamless transition of care between different resources. A patient with long term conditions moves through different resources: consultations, rehabilitation, hospitalization (either for health care reasons or, at other times for social ones), and emergency care.
IT IS A GOOD EXAMPLE OF ORGANISING AND DEIVERING CARE KEEPING THE PATIENT AT THE CENTRE.
